The Jefferson County Department of Health is seeking a Population Health Manager to oversee the strategic development, implementation, and evaluation of all population health programs. This position provides operational and administrative leadership for a diverse group of initiatives aimed at addressing the social determinants of health and reducing health disparities across the community. Key areas of oversight include injury prevention (such as opioid overdose and gun violence), maternal and child health, chronic disease and cancer prevention, language access services, and integrated social services. The manager ensures compliance with federal and state grant requirements, optimizes resource allocation, and fosters cross-sector collaborations with healthcare systems, academic institutions, governmental agencies, and community-based organizations.COMPENSATION & BENEFITS:

MINIMUM QUALIFICATIONS:
The following are job-related qualifications that are required for employment consideration for this position:
Bachelor’s degree from an accredited college or university.
Experience in population health, public health, or community health programs, including program development and implementation, grant or budget management, data evaluation and reporting, and stakeholder or community engagement.
Experience managing a professional team, including performance appraisals, hiring, and disciplinary actions.
PREFERRED QUALIFICATIONS:
The following are job-related qualifications deemed desirable by Jefferson County Department of Health. These qualifications may be considered by the hiring agency when reviewing applications and inviting candidates to participate in subsequent steps in the selection processes.
Master's degree in Public Health (MPH), Public Administration (MPA), Health Administration (MBA or MSHA), Social Work, Nursing, or a public health or healthcare-related friend from an accredited university or collage.
Experience in a leadership role within a healthcare or public health setting (e.g., creating or editing policies and procedures, leading strategic health programs, high-level communication).
Experience managing state and/or federal public health grants.
Experience developing and maintaining a budget.
TYPICAL JOB DUTIES
Lead the planning, implementation, and evaluation of comprehensive overdose prevention strategies funded through federal and state grants. Collaborate with community partners, healthcare providers, and internal teams to expand access to harm reduction services, treatment resources, and public education. Ensure compliance with grant requirements and track program outcomes to demonstrate impact.
Develop strategic funding plans for opioid settlement and grant resources to address substance use disorder prevention, treatment, and recovery. Engage with stakeholders to ensure community input and alignment with public health priorities. Monitor expenditures, assess outcomes, and provide transparent reporting on the use of funds.
Oversee initiatives aimed at improving maternal and infant health, with a focus on reducing disparities in birth outcomes. Analyze epidemiological data to identify trends and develop targeted interventions. Collaborate with healthcare systems, social service agencies, and advocacy groups to support prenatal care access, maternal health education, and early childhood development
Implement evidence-based interventions and cross-sector collaborations to reduce firearm-related injuries and deaths. Lead data collection, stakeholder engagement, and community outreach to support violence prevention efforts. Evaluate program effectiveness and adapt strategies in response to community needs and emerging trends.
Direct the planning and implementation of community health programs focused on chronic disease prevention and control, including diabetes, hypertension, and obesity. Partner with clinical providers, schools, and community organizations to promote health screenings, education, and lifestyle interventions. Monitor performance metrics and health outcomes to guide continuous improvement.
Develop, monitor, and manage budgets for multiple population health initiatives. Ensure responsible stewardship of resources through accurate forecasting, fiscal tracking, and grant compliance. Prepare budget reports and funding proposals to sustain and expand program capacity.
Oversee the integration of social service supports, such as housing referrals, transportation assistance, and benefits navigation, into public health programs. Collaborate with community-based organizations and governmental agencies to address.
Provide leadership and oversight to the language services team, ensuring equitable access to public health programs for individuals with limited English proficiency (LEP). Implement policies and procedures for interpretation and translation services. Monitor service delivery to ensure cultural and linguistic appropriateness across all population health initiatives.
PHYSICAL DEMANDS:
Job is primarily sedentary, involving sitting for long periods of time, but may involve occasional walking or standing for brief periods. Job may also involve occasional light lifting of items or objects weighing up to 25 lbs.WORK ENVIRONMENT:
Work is conducted almost exclusively indoors in an office setting and involves use of standard office equipment, such as computer, phone, copier, etc. May be exposed to a variety of adverse working conditions, including possible exposure to bodily fluids and other bio-hazard materials (e.g., urine, sweat, saliva, blood, etc.), and possible exposure to contagious or infectious diseases.
